Endotracheal Tube Connection
In a recent event the endotracheal tube adaptor became disconnected from the tube causing the patient to be disconnected from the ventilator.
Staff caring for patients should be aware that:
- The establishment of a firm connection between the adaptor and endotracheal tube reduces the likelihood that the junction will become disconnected, thus reducing the potential for adverse events.
Safe practices to prevent this type of event:
- The manufacturer packaging recommends to “Firmly insert the 15mm connector prior to use”.

- Prior to insertion, and post endotracheal cuff inflation check, establish a firm connection between the adaptor and the endotracheal tube.
