The following list shows the supervisor, with the employee(s) in your area who need to update their badge. Employees have now received three email notifications to do so (see details below this list). Please remind them to update their badge by Nov. 6.

This is a reminder that technology upgrades require you to update your UI Health Care photo identification badge. Please visit the UI Health Care Badge Center in General Hospital C110 (Elevator B, Level 1) by Nov. 6, 2019.*
Before visiting the Badge Center, visit the Nursing Recruitment desk, T100-Z (Elevator B, Level 1), to obtain a new Photo Identification Badge Request form. This form is required and must be created and signed by your HR representative.
Please note: A new photo will be taken.
The Badge Center is open 7:30 a.m. to 4:30 p.m., Monday through Friday (excluding university holidays). See guidelines for UI Health Care badges.
