"Conducting a Hazard Analysis" Course This Month

EHS will present “Conducting a Hazard Analysis” this month via Zoom; sign-up information is included in the link above. The course is required for those applying for the Laboratory Safety Certificate. We will discuss how to conduct a hazard assessment as well as different ways of organizing the assessment process.

Poliovirus Containment Survey Forthcoming

To help the University of Iowa (UI) fulfill requirements set forth by The U.S. National Authority for Containment of Poliovirus, EHS biosafety staff will be sending out a poliovirus containment survey to UI laboratories. The survey questions are intended to identify laboratories that possess materials that may contain poliovirus.

New Safety Resource

A new Field Activity Hazard Assessment document is available for use.  This document outlines common environmental, health, and safety hazards as well as safety precautions and strategies needed to minimize risk while in the field.  The assessment is located on the Laboratory Safety Resources and Training webpage under Documents.

DEA Form

DEA Form 41 for Controlled Substance Disposal

If you’re disposing of a controlled substance, you must complete the most current approved DEA Form 41. The document expiration date is printed in the upper right-hand corner.  If you have saved a copy of the PDF for use locally, it’s better to obtain the most current form directly from the DEA. The DEA Form 41 can be found on the DEA Diversion Control website in the Reporting Forms area of DEA Forms & Applications.
